Life Certificate Forms

Since 1 February 2012, the Istituto Nazionale della Previdenza Sociale (INPS) has partnered with Citibank to provide pension benefit payments for pensioners, who like yourself, reside abroad.

To ensure correct and proper payment of applicable pensions, each and every year applicants must complete a Life Certificate Form. (Yellow form)

For assistance in compiling and authenticating the relevant application form, applicants are encouraged to contact your local INCA office, who will ensure the process is completed promptly and accordingly.

In exceptional circumstances, such as in the case of hospitalized retirees or nursing home residents unable to move independently, the authentication can be performed by an official from the hospital or nursing home itself. In these circumstances, and to ensure that payments are made correctly and appropriately, an alternative application form will need to be completed. (Green form)
